Medtronic details future of AI-assisted surgery

Medtronic is advancing the role of artificial intelligence in surgery with an FDA-approved algorithm designed to track surgical instruments in real time. The company announced the launch of Touch Surgery Aide, a new computing platform intended to bring digital assistance directly into the operating room. The technology represents the first AI algorithm for surgical use to receive clearance from the Food and Drug Administration.

According to George Murgatroyd, vice president and general manager of digital technologies at Medtronic, the technology addresses a significant gap in current surgical practices. He noted that surgeons often have more AI assistance in their cars than they do while performing complex procedures.

The platform, which was demonstrated at the Society of Robotic Surgery conference in Florida, runs multiple AI models simultaneously on the live procedure feed. Its first application, called IEP (Instrument Exit Point), works with the Hugo soft tissue robotic surgery system. The algorithm tracks the surgical instrument and notifies the surgeon when the tool leaves their field of view. Medtronic describes this function similarly to a blind spot in a vehicle, where the AI provides a visual guide to the instrument’s location.

“We really do see a future where, in the background during cases, AI is running to support surgeons when they need it, and they don’t always know when they need it,” Murgatroyd said.

Touch Surgery has been deployed in over 1,500 operating rooms globally, where it has provided pre-operative case reviews, automated benchmarking, and global collaboration tools. The new Aide platform adds the capability to support decisions during the procedure itself.

Medtronic highlighted that a major barrier to AI adoption in surgery is the lack of computing infrastructure in many procedure rooms. The company stated that even in some of the most expensive real estate in the world, operating rooms often lack the hardware required to run advanced AI models in real time.

The Aide launch addresses this by providing the necessary computing power. The IEP algorithm is classified as a medical device data system. Because the visual information is displayed on screens during surgery and is intended to guide the clinician, it must adhere to strict regulatory pathways.

“Anything we present to surgeons visually, even if it’s providing visual assistance, is classified as a medical device,” Murgatroyd explained. “It’s on the screens as they operate, and we’re providing information to them that is important to them. So it has to follow a regulatory pathway because no patient would want surgeons to have unregulated, untested AI running whilst they’re being operated on.”

While the current technology focuses on visual guidance, Medtronic anticipates a stepwise progression. Future iterations may develop into real decision support and, eventually, potential automation. The company views this as the beginning of a broader shift toward integrating AI into surgical workflows.

The organization believes that making this innovation scalable is critical because surgical outcomes remain variable globally. By opening the platform, Medtronic aims to accelerate the delivery of life-changing tools to surgeons worldwide.
