The healthcare industry is witnessing a surge in advanced therapies, including transplants, cell, and gene treatments, offering hope for conditions like cancer and spinal muscular atrophy. However, this growth presents challenges for payer organizations, who must handle complex care plans and escalating costs. Over the past decade, the rapid expansion of these therapies has transformed the treatment environment, but it has also introduced unprecedented financial and operational complexities for health plans.

Working through the Complexities of Advanced Therapies

A recent HIMSS webinar, “Successful Strategies for Handling Growth in Transplant, Cell, and Gene Therapies”, addressed these challenges. Lisa Latts, MD, Chief Medical Officer at Optum’s Whole Health Solutions, emphasized the need for deeper integration across clinical care, decision-making, and patient support. These therapies demand a more coordinated approach, ensuring patients receive the right care at the right time. Latts highlighted that the non-linear nature of these care journeys requires a holistic view, integrating not only medical treatment but also social and logistical support to ensure successful outcomes.

Latts, alongside Michael O’Day, Vice President of Product Management at Optum Transplant Solutions, and Marjorie Chen, Senior Director of Product at Optum, highlighted the importance of combining clinical guidance, active care management, and access to specialized care centers. This approach not only improves patient outcomes but also helps manage the financial burden on health plans. They stressed that the integration of data analytics and technology can further enhance care coordination, enabling real-time decision-making and personalized treatment plans.

Transplant Trends and Rising Costs

The transplant environment is evolving with increased organ donation rates, relaxed donor criteria, and improved complication management. Perfusion devices, which preserve donated organs, have significantly boosted transplant utilization. Additionally, xenotransplantation, using genetically modified animal organs, is expanding the organ pool. These advancements have not only increased the number of viable organs but also improved the success rates of transplants, leading to higher demand for these procedures.

However, these advancements come with a price tag. O’Day noted that the average transplant cost is approximately $892,000, and with rising utilization, health plans face substantial financial exposure. Even minor changes in utilization can significantly impact overall costs. He further explained that the increased complexity of post-transplant care, including lifelong immunosuppression and monitoring, adds to the long-term financial burden on health plans. The growing adoption of xenotransplantation, while promising, introduces additional regulatory and ethical considerations that further complicate cost management.

The Promise and Challenges of Gene and Cell Therapies

Gene and cell therapies are transforming treatment for previously untreatable conditions. Chen explained how these therapies correct diseases at the genetic level, offering potential cures. However, their complexity and high costs pose challenges for health plans. She detailed how these therapies often require personalized treatment protocols, extensive patient monitoring, and specialized infrastructure, all of which contribute to their high costs. Additionally, the long-term efficacy and safety of these therapies are still being studied, adding uncertainty to their financial impact.

These therapies require sophisticated contracting approaches, including specialized provider networks and innovative pricing models. The focus is on managing costs while ensuring access to life-changing treatments. Chen discussed the importance of value-based agreements, such as outcomes-based contracts, which tie payment to the effectiveness of the treatment. She also highlighted the role of reinsurance and full-risk transfer models in mitigating financial risk for health plans, ensuring that the cost of these therapies does not become prohibitive for patients or payers.

Coordinated Care: The Key to Cost-Effective Treatment

The experts agreed that the healthcare system already possesses the tools to treat complex diseases. The missing piece is effective coordination. By providing care coordination, patient education, and clinical oversight throughout the treatment journey, health plans can improve outcomes and manage costs. They emphasized that coordinated care involves breaking down silos between different healthcare providers and ensuring seamless communication across all stages of treatment. This includes pre-authorization, treatment delivery, and post-treatment follow-up, all of which are critical to achieving the best possible outcomes.

This coordinated approach is key as the industry continues to adopt advanced therapies. It ensures that patients receive the full benefit of these treatments while addressing the financial sustainability of health plans. The experts concluded that by leveraging technology, data analytics, and innovative payment models, health plans can work through the complexities of advanced therapies and make them accessible and affordable for all stakeholders.

As the healthcare system evolves, the focus on coordination and integration will be vital to making these new therapies accessible and affordable for patients and payers alike.
